A Clinical Study for the Complication of the Post Gastric Surgery


Abstract
A clinical study on 505 patients with gastric disease who had been admitted and received Gastric resection at Dept. of the Surgery Korea University Hospital from January 1978 to December 1982. had been analyzed and results were summarized as follows:
1) The ratio between male and female was 2.3: 1 and incidence of the peak age was 51~60 years.
2) During this 5 year, subtotal gastrectomy with B-II was performed in 243 cases, and also were performed Trunkal vagotomy with pyloroplasty in 99, Gastrojejunostomny in 53, simple closure in 48.
3) The distribution of complications were Pneumonia 18.9%, Intraluminal bleeding 17.5%, wound infection 11. 1%, Atelectasis 9.8%, Pleural effusion 7. 0%, Thrombophlebitis 7. 0%, Jaundice 3.5%, Sepsis 3.5%, Duodenal stump leakage 2.8%, Intraabdominal abscess 2.8%, Metabolic alkalosis 2.8%, Renal failure 2.1% in the order of frequency.
4) Total number of complicated patients was 101(28.3%).
5) The incidence of complications according to the disease was 33.3%in Gastric ulcer, 20.6% in duodenal ulcer, 14.7% in gastric cancer.
6) The incidence of complications according to the technique of operation were more higher wish simple closure, subtotal gastrectomy with B-II than other procedure.
7) Hemoglobin and Hematocrit were not significant findings between the complicated and non-complicated patients.
8) Mortality was 3%.
9) Most common causes of the death were bleeding, sepsis, pneumonia in the order of frequency.
